Kevin Anger

Kevin Anger received his PharmD Degree in 2004 from Northeastern University, followed by residency training at Brigham and Women’s Hospital (BWH) in Boston M.A. Kevin is a Pharmacy Supervisor overseeing clinical pharmacy Services at BWH in Boston M.A. In his current role, Kevin oversees the clinical services for critical care, cardiology, and oncology as well as educational/professional development activities in the pharmacy department. Kevin’s background comes from 10 years as a clinical pharmacy specialist in the medical intensive care unit, and 6 years of supervisor work overseeing investigational drugs and clinical services at BWH. Kevin is actively involved in education as a PGY-1 and PGY-2 residency preceptor at BWH, and student preceptor for Northeastern University's Bouve' College of Pharmacy. Kevin is a board certified pharmacotherapy specialist, and is involved in outcomes based research in critical care pharmacotherapy and has authored several research manuscripts, review articles, and book chapters. Kevin maintains membership with several professional organizations including the American Society of Health Systems Pharmacists (ASHP) where he serves on the Educational Steering Committee for the Section of Clinical Scientists and Specialists and Faculty member for the ASHP Critical Care Board Review Course. Kevin is a frequent speaker on educational topics at local and national meetings on topics related to critical care and research pharmacy services.

Angela Bingham

Angela L. Bingham, PharmD, BCPS, BCNSP, BCCCP, FASHP, FASPEN, FCCP, is Vice Chair and Associate Professor of Clinical Pharmacy as well as PGY2 Critical Care Pharmacy Residency Program Director at the Philadelphia College of Pharmacy. Dr. Bingham earned her Doctor of Pharmacy degree from the South Carolina College of Pharmacy in Columbia, South Carolina; completed a pharmacy practice residency at The Johns Hopkins Hospital in Baltimore, Maryland; and completed a specialized residency in critical care at The Regional Medical Center at Memphis in conjunction with The University of Tennessee Health Science Center. She practices as a Critical Care Clinical Pharmacy Specialist at Cooper University Health Care in

Camden, New Jersey. Dr. Bingham’s practice, teaching, and research activities are focused on pharmacotherapy in critical illness including nutrition support therapy. She is active within ASHP serving as a member of the Council on Education and Workforce Development. Within the American Society for Parenteral and Enteral Nutrition, she has served on task forces for adult critical care nutrition guideline revisions, parenteral nutrition appropriateness consensus recommendations, and Standards of Practice for Nutrition Support Pharmacists. Dr. Bingham is a Board Certified Critical Care Pharmacist, Board Certified Nutrition Support Pharmacist, and Board Certified Pharmacotherapy Specialist.

Christopher CarterDr. Carter is the current Clinical Manager for Pharmacy Services at SSM Health St. Clare Hospital - Fenton (Missouri) and is a past clinical pharmacy specialist in critical care for the mixed intensive care unit at the hospital. He is also an Adjunct Experiential Assistant Professor with St. Louis College of Pharmacy. Dr. Carter received his PharmD from the University of Kansas and completed PGY-1 and PGY-2 Critical Care residency training at Barnes-Jewish Hospital in St. Louis, Missouri. Quinn Czosnowski

Quinn A. Czosnowski, PharmD, BCCCP, is Critical Care Clinical Pharmacy Specialist at Indiana University Health - Methodist Hospital in Indianapolis, IN. Dr. Czosnowski graduated with his Doctor of Pharmacy from Purdue University and completed a PGY-1 residency at Methodist University Hospital in Memphis, TN and a PGY-2 in Critical Care at The Regional Medical Center at Memphis. Brian Dee

Brian Dee, PharmD, BCCCP, BCNSP, FCCM is a Critical Care/Nutrition Support Clinical Pharmacy Specialist at The University of Texas MD Anderson Cancer Center in Houston, Texas. Heather Draper

Heather Draper, PharmD, BCPS, is a Clinical Pharmacist in Emergency Medicine at Trinity Health Saint Mary’s in Grand Rapids, Michigan. Dr. Draper has twice received the Excellence in Teaching Award from the Student Government Association, the Most Influential Professor and the Preceptor of the Year Awards at the University of Tennessee and was awarded the Innovative Pharmacy Practice Award from the Tennessee Pharmacists Association. She has been certified as an instructor by the American Heart Association for Basic Life Support and Cardiopulmonary Resuscitation, Advanced Cardiac Life Support, and Pediatric Advanced Life Support. Dr. Draper is a former chair of the American Society of Health System Pharmacists (ASHP) Section Advisory Group on Emergency Care and an author of the ASHP Guidelines on Emergency Medicine Pharmacist Services. Her research interests include examining the role and impact of the pharmacist in the Emergency Department and in a variety of resuscitation settings.

Tyler HallerTyler Haller completed his critical care residency training at Methodist University Hospital in Memphis, TN. His current role is Clinical Pharmacy Specialist in the Neurocritical Care Unit at Barrow Neurological Institute within St. Joseph’s Hospital and Medical Center in Phoenix, Arizona. Dr. Haller acts as the primary pharmacy resource for determination of best practice pharmacotherapy and order options, safety and monitoring considerations, and accreditation activities for SJHMC’s neurology and neurosurgery patient populations. He precepts residents in the Neuro ICU as well as a Neurology rotation for students from University of Arizona, Midwestern University, Creighton University and Purdue University.

Mojdeh HeavnerDr. Mojdeh Heavner is an Associate Professor and Vice Chair for Clinical Services for the Department of Pharmacy Practice and Science at the University of Maryland School of Pharmacy in Baltimore, MD and serves as the Coordinator for the Pharmacotherapy PGY1/PGY2 Residency Program. She completed her undergraduate work at the University of Maryland College Park and Pharm.D. degree at the University of Maryland School of Pharmacy. Thereafter, Dr. Heavner completed residencies in pharmacy practice and critical care/solid organ transplant at Yale New Haven Hospital in Connecticut. After residency, Mojdeh stayed on at Yale New Haven as the Medical ICU Clinical Pharmacy Specialist, Supervisor of Clinical Pharmacy Services, and Director of the Critical Care Residency Program. She attained board certification in pharmacotherapy in 2009 and critical care in 2015. Mojdeh practices in a 29-bed Medical ICU at the University of Maryland Medical Center. Her practice and research interests include sleep and delirium in the ICU, PKPD in critical illness, and recently, she has been involved in scholarship and education related to the COVID-19 pandemic.

Mary M. Hess, PharmD, BSPharm, BCCCP, FASHP, FCCM, FCCP, is Associate Dean for Student Affairs and Professor of Pharmacy Practice at the Jefferson College of Pharmacy, in Philadelphia, Pennsylvania. Dr. Hess's training and education include a Bachelor of Science in Pharmacy degree from the University of Missouri – Kansas City; Doctor of Pharmacy from the St. Louis College of Pharmacy; Critical Care/Nutrition residency and a Critical Care Fellowship at the University of Tennessee. Dr. Hess's career has been primarily in academia with a clinical practice in critical care pharmacy. She has built and directed health-system clinical pharmacy services at two major health systems, established new post-graduate pharmacy training programs, directed both PGY 1 Pharmacy and PGY2 Critical Care post-graduate residency programs, and developed innovative student internship programs. Dr. Hess has served professional organizations in a variety of roles including Chair of the Commission on Credentialing, Critical Care Network Facilitator and Clinical Specialist and Sciences leadership within ASHP, as a member of the BPS Petition Committee for Critical Care Board Certification and ASHP Critical Care Board Review course faculty and as a Board member for an ASHP Accredited Technician training program.

Benjamin Hohlfelder

Benjamin Hohlfelder, PharmD, BCPS, BCCCP, is a Critical Care Pharmacy Clinical Specialist at the Cleveland Clinic. His clinical practice is centered in the cardiothoracic surgery ICU, including patients undergoing heart and lung transplant, ventricular assist device implantation, and alternative modes of mechanical circulatory support. He completed a PGY-1 Pharmacy Residency and PGY-2 Critical Care Pharmacy Residency at Brigham and Women's Hospital in Boston, MA. He is board certified in both Pharmacotherapy and Critical Care Pharmacy. Areas of research interest include anticoagulation in mechanical circulatory support, management of pain, agitation and delirium, and treatment of venous thromboembolism. Most recently, he has served as faculty for the 2019 ASHP Literature Studies modules in Pharmacotherapy and Critical Care, and presented for the Intensive Studies session at the 2018 ASHP Midyear Clinical Meeting.

Lauren Igneri

Lauren A. Igneri, PharmD, BCPS, BCCCP is the Clinical Pharmacy Specialist in Critical Care at Cooper University Healthcare (CUH) in Camden, NJ. Dr. Igneri earned her Doctor of Pharmacy degree from the Ernest Mario School of Pharmacy at Rutgers University. She went on to complete a PGY1 Pharmacy Practice Residency at Temple University Hospital in Philadelphia, Pennsylvania followed by a PGY2 Critical Care Specialty Residency at University of the Sciences in Philadelphia with her main practice site at CUH. Following residency, Dr. Igneri was the Critical Care Clinical Pharmacy Specialist at St. Mary Medical Center (SMMC) in Langhorne, Pennsylvania and served as co-chair of the Trinity Health Clinical Pharmacy Committee to develop Transforming Operations initiatives, streamlining clinical and cost-savings activities across an approximately 90-hospital health system. She was an active member of the institutional review board and served as an Advance Pharmacy Practice Experience preceptor for students from University of the Sciences, Jefferson College of Pharmacy, and Rutgers University while at SMMC. Since returning to CUH, Dr. Igneri continues to be involved in numerous multidisciplinary committees to improve the care of critically ill patients. She serves as a primary preceptor for the critical care residents’ rotation, research, and continuing education experiences.

Dr. Igneri has been the primary or co-investigator on numerous research investigations including anticoagulation, anticoagulation reversal, thrombolysis in venous thromboembolism, glycemic control, pharmacokinetics, and sedation practices in critically ill patients, among others. She has been invited to present regularly at local, regional, and national meetings. Dr. Igneri serves as a peer-reviewer for the Journal of Pharmacy Practice and Annals of Pharmacotherapy. Dr. Igneri is an active member of the Society of Critical Care Medicine and contributes regularly within the Clinical Pharmacy and Pharmacology Communications Committee. In 2019, she was elected Member at Large of the Women in Critical Care Knowledge Education Group. Additionally, Dr. Igneri is a member of the American College of Clinical Pharmacy and the American Society of Health-System Pharmacists. She has served as faculty creating educational materials for ASHP Board Certification Resources since 2016.

Patricia Kienle, M.P.A., BCSCP, FASHP, is the Director of Accreditation and Medication Safety for Cardinal Health Innovative Delivery Solutions. She received her pharmacy degree from the Philadelphia College of Pharmacy and Science and a Masters in Public Administration from Marywood University in Scranton, Pennsylvania. She is Board Certified as a Sterile Compounding Pharmacist. She completed an Executive Fellowship in Patient Safety from Virginia Commonwealth University in Richmond, Virginia, and is an Adjunct Associate Professor at Wilkes University in Wilkes-Barre, Pennsylvania. She has served on the Board of Directors of the American Society of Health-System Pharmacists and as President of the Pennsylvania Society of Hospital Pharmacists. She is a Fellow of ASHP, was named Pharmacist of the Year by PSHP, the recipient of the Distinguished Achievement Award in Hospital and Institutional Practice from the American Pharmaceutical Association Academy of Pharmacy Practice and Management, the Distinguished Leadership Award from ASHP, the ASHP John W. Webb Lecture Award, and the Thomas S. Foster Award from the United States Pharmacopeia. She has served on the Pharmacotherapy Specialty Council of the Board of Pharmacy Specialties, the Pennsylvania Patient Safety Authority, the Hospital Professional and Technical Advisory Committee of The Joint Commission, and on the Board of Governors of the National Patient Safety Foundation. She is a current member of the USP Compounding Expert Committee and chairs the Subcommittee on Hazardous Drugs. Patti is the author of Compounding Sterile Preparations: ASHP’s Visual Guide to Chapter <797> video and Companion Guide, co-author of Meeting Accreditation Standards: A Pharmacy Preparation Guide, and author of The Chapter <795> Answer Book, The Chapter <797> Answer Book, and The Chapter <800> Answer Book. With over 500 invited presentations and 80 publications, she has special interests in promoting medication safety, compounding sterile preparations, accreditation, and regulatory issues.

Daniel MalcomDr. Malcom is an Associate Professor and Chair of the Department of Pharmacy Practice. He obtained his Doctor of Pharmacy degree from the University of Georgia College of Pharmacy in 2008, then completed a PGY-1 Pharmacy Practice Residency and a PGY-2 Specialty Residency in Critical Care at the University of Tennessee Medical Center in Knoxville, Tennessee (affiliated with the University of Tennessee College of Pharmacy). Dr. Malcom’s research has been accepted for oral presentations and poster sessions at regional, national, and international meetings including the American Society of Health Systems Pharmacists (ASHP) Midyear Clinical Meeting, the International Conference of the American Thoracic Society (ATS), the Society of Critical Care Medicine (SCCM) Annual Congress, and the American Association of Colleges of Pharmacy (AACP) Annual Meeting. Dr. Malcom was also recognized as an American Association of Colleges of Pharmacy (AACP) Teacher of the Year in 2013, Kentucky Society of Health System Pharmacists (KSHP) Mentor of the Year in 2017, and as an AACP Emerging Teaching Scholar in 2019. He also completed AACP’s Academic Leadership Fellows Program (ALFP) as a part of Cohort 14 in 2017-18. Dr. Malcom also maintains active memberships in several professional pharmacy organizations and has served as an item-writer for the North American Pharmacist Licensure Exam (NAPLEX). Additionally, he is an Associate Editor for the American Journal of Pharmaceutical Education (AJPE), the official journal of AACP.

Krystal MoormanKrystal Moorman, PharmD, BCPS, is Director of Experiential Education and Clinical Assistant Professor at the University of Utah College of Pharmacy in Salt Lake City. Dr. Moorman earned a Doctor of Pharmacy degree from the Medical University of South Carolina College of Pharmacy in Charleston and completed PGY1 Pharmacy Practice and PGY2 Drug Information residencies at the Medical University of South Carolina. Dr. Moorman’s interests include drug information and medication safety. She is an active member of the Utah Society of Health-System Pharmacists---serving as President and now Immediate Past President. Dr. Moorman has served as Chair and member, ASHP Program Committee; Delegate, 2015 ASHP House of Delegates; and Alternate Delegate, 2016 ASHP House of Delegates. Dr. Moorman has served on the ASHP Advisory Group for Preceptor Development and on the Council on Education and Workforce Development.

Rachel SwopeRachel Swope, PharmD, BCCCP is a Clinical Pharmacy Manager at Norton Healthcare, a five hospital community health system in Louisville, Kentucky. Dr. Swope completed her Doctor of Pharmacy at the University of Kentucky College of Pharmacy in Lexington, KY. Paul SzumitaPaul M. Szumita is the Director of Clinical Pharmacy Services and Program Director of the postgraduate year 2 critical care pharmacy residency at Brigham and Women¹s Hospital in Boston, Massachusetts. Paul is a fellow in the American College of Critical Care Medicine and the American Society of Health-System Pharmacy. Paul has helped develop and is responsible for managing clinical programs aimed at optimizing pharmacotherapy and improving patient outcomes. He has an active role in clinical practice, education, clinical research, and guideline development/implementation with a focus inpatient glucose management, ICU pain, agitation and delirium, and hemodynamics in shock states. He has more than 70 peer-reviewed publications, and he has served on/currently serves on several committees focused on improving clinical practice at the local, regional, and national level, including the program advisory board for the ASHP Critical Care Pharmacy Specialty Board Examination Review Course, guideline writing committee for the 2018 Society of Critical Care Medicine guidelines for the management of pain, agitation, delirium, immobility and sleep disruption, and serves on the taskforce to revise the Society of Critical Care Medicine Guidelines stress ulcer prophylaxis in critically ill patients.

Heather Torbic

Heather Torbic, PharmD, BCPS, BCCCP, is Medical Intensive Care Unit Clinical Specialist at the Cleveland Clinic in Cleveland, Ohio. Dr. Torbic received a Doctor of Pharmacy degree from Northeastern University in Boston, Massachusetts; completed a PGY1 Pharmacy Practice Residency at Boston Medical Center; and, a PGY2 Critical Care Residency at Brigham and Women's Hospital in Boston.

In addition to patient care, Dr. Torbic is actively involved with student and resident training and is the program director for the PGY2 Critical Care Residency Program at the Cleveland Clinic. She is also actively involved in research, which includes acute respiratory distress syndrome and sedation/analgesia/agitation in critically ill patients. Dr. Torbic is an active member of the American Society of Health-System Pharmacy and serves as faculty for the ASHP BCCCP review course. She is also an active member of the Society of Critical Care Medicine and its CPP education section.

Andrew WebAndrew Webb is a board-certified neurocritical care clinical pharmacist at Massachusetts General Hospital. He is originally from Rhode Island and completed his undergraduate studies and Doctor of Pharmacy at the University of Rhode Island. He then completed his PGY1 pharmacy residency at Mayo Clinic in Rochester, MN followed by a PGY2 in critical care at Oregon Health & Science University in Portland, OR. His clinical and research interests include traumatic brain injury, infectious diseases in critical illness, and optimizing the medical management of neurocritically ill patients. He is active in the American College of Clinical Pharmacy and the Neurocritical Care Society and serves on the research committee for the ACCP Critical Care PRN. He also serves on research and quality improvement committees within the MGH Departments of Pharmacy and Neurology.
